To apply a claim you will need to fill in all necessary information from the purchase receipt and warranty card. Please check warranty period applicable for the claimed product and conditions of warranty.
For the claim acceptance and its settlement it is necessary to fill in all obligatory items (marked with one star).
The maximum size of attachments must not exceed 20 MB
Textual description of the claimed product, e.g. Blix shower enclosure, transparent glass, white frame.
Serial number is placed on product package, on product itself or in warranty card. If there is no serial number , please, enclose the copy of the purchase receipt.
Upon successful submitting the form, you will receive a confirmation message to your mail with the complaint number under which it will be registered with RAVAK a.s.. Use this number for further communication regarding the complaint.
